Subject: Ohio Bail Bond Agent License Acquired Through Deception
Mr. Qualls received his WV license on 21 March 2007 and then through reciprocity received his Ohio Bail License on 26 March 2007, five (5) days later. On the same day Mr. Qualls approached Mr. John Craven of Cravens General Agency, Springfield, OH and presented an Ohio Bail License. Mr. Craven, assuming that Mr. Qualls was legal, appointed Mr. Qualls as a bail bond agent for his company to post bonds in West Virginia and/or Ohio. Note: Appointment was on same day, 26 March 2007. See Exhibits “A” & “B”.
Mr. Qualls then asked Mr. Craven for skip files to work so that he could kick start his retrievals business. Mr. Qualls began doing retrieval work and started advertising himself as a Bail Agent with the courts in Gallia County and Meigs County, OH.
On 19 April 2007, during a routine visit by myself to Mr. Craven’s office, Mr. John Craven and I were discussing Terry Qualls and his license status. I then pulled down his West Virginia Bail License and noticed that his address was 1432 Brick School Rd., Gallipolis, OH 45631, Exhibit “A”. This seemed out of place - Mr. Qualls having a Non-Resident WV license with an Ohio address?
Based on our concerns about Mr. Qualls and his WV and OH Bail Bond License, John Craven and I called The West Virginia Offices of the Insurance Commissions and asked how Mr. Qualls obtained a WV license with an Ohio address. The lady at the department told us that during testing Mr. Qualls did an address change request to Ohio. She stated that West Virginia allows individuals to do an address change during “Test Date” if their address has changed since their application for licensing was filed and/or accepted.
Mr. Craven and I then called The Ohio Department of Insurance (ODI) and spoke with Ms. Rhonda Milner and asked her about Mr. Terry Qualls. She knew of Mr. Qualls immediately. Mr. Craven then asked Ms. Rhonda Milner since we suspect Mr. Qualls cut corners and played a possible loop hole in obtaining his Ohio Bail Bonding License, if this could come back to his company in the future, if there was an investigation. Ms. Miller told him this was very possible.
Based on the discussions above, Mr. Craven contacted Mr. Qualls and informed him of his concerns with his bail bond license and terminated Mr. Qualls appointment on 11 April 2007, Exhibit “B”.
During Mr. Qualls time working in the office of Craven Bail Bonds, he had discussed ways for other Craven employees to obtain their OH Bail License without going thru all the hassle of taking the mandatory schooling, testing, school cost, time involved and then waiting six (6) months to start posting bonds. He expressed that all they needed to do was get a WV License and then through reciprocity obtain an OH license and start working immediately. In other words, Mr. Qualls felt that what he had done was something other future bail agents may consider to get in the system quicker. This too was a concern of Mr. Craven.
Another Address Change
Mr. Qualls, sometime after his appointment being pulled by Mr. Craven, contacted The West Virginia Offices of the Insurance Commissions and had his address changed “Back From” 1432 Brick School Rd, Gallipolis OH 45631 “Back To” 18240 River Rd, West Columbia, WV 25287, which is the address of a Ms. Lisa Davern. Sometime during this period he then contacted ODI and had his address changed there also, Exhibit “D”. Again, he appears to be working the system to keep his Non-Resident WV License appear legitimate.
Note: 18240 River Rd, West Columbia, WV 25287 is incorrect. The Address is 18240 Ohio River Rd, West Columbia, WV 25287. Through his rush deception attempt he failed to note that OHIO was missing from the address he submitted to ODI, he even failed to report the numbers correctly to WV., Exhibits "C", "D", "E".
Again, Mr. Qualls approached Mr. Craven and presented his OH/WV License as being legitimate and Mr. Craven (again) granted him an active appointment with The North River Insurance Company, on 10 May 2007, Exhibit “D” and “E”. Note: Keep in mind, Mr.Qualls currently, and has for sometime, lived @ 1432 Brick School Rd, Gallipolis OH 45631
In April 2007, Mr. Qualls opened a business account with The National City Bank in Springfield, OH, under the name of “JTQ United LLC, Anytime Bail Bonds, (740) 367-7740, 1432 Brick School Rd., Gallipolis, OH 45631”, his current OH address, see Exhibit “F”.
Note that he even listed the business as a Limited Liability Company (LLC), which it is very doubtful.
Question: Why open a business account in Springfield, OH when you live in West Virginia (or is it Gallipolis, OH) and why have a business account in Ohio but not in West Virginia, when your primary reason to obtain a WV License was to do work for Craven Bail Bonding Agency and post bonds in West Virginia? Seems like a good business plan to open up a Business Account 5 counties away from your home or even in another state, if you planned on maybe writing a few bad checks, see section “Things Start Coming Unraveled” below.
In the last week, Mr. Qualls has posted bonding cards with the jails in Gallipolis, OH; Middleport, OH; Pomeroy, OH and attempted to qualify in Athens county through the postal service but was declined. His bonding cards read: Anytime Bail Bonds, All States, Accepts Visa/MC, Mr. Terry Qualls Jr., Agent. Toll Free: 877-367-7116, 740-367-7100 Fax: 740-367-7106, Exhibit “G”
The telephone numbers above are registered with Anytime Bail Bonds, 1432 Brick School Rd, Gallipolis, OH 45631, which again is where Mr. Qualls lives.
Question: How does Mr. Qualls work as an agent for Craven Bail Bonds and display business cards reading “Anytime Bail Bonds”? It appears, these cards represent nothing more than a name and someone attempting to circumvent or corrupt the bail bonding system and portray themselves as a legitimate bonding company.
Things Start Coming Unraveled
On 29 May 2007, I was contacted by a Mr. Paul Hoover, who is/was Mr. Qualls skip retrieval partner, regarding his dealings with a Mr. Terry Qualls. Mr. Hoover asked for my assistance with contacting Mr. Craven and discussing Mr. Qualls. He felt that Mr. Qualls was deceiving him and Mr. Craven and felt that he needed to talk with Mr. Craven about his concerns. Mr. Hoover currently lives in WV with Ms. Lisa Davern @ 18240 Ohio River Rd, West Columbia, WV 25287.
I called Mr. Craven and he agreed to meet with me and discussed the situation concerning Mr. Qualls. On 31 May 2007, I met with Mr. Craven and during our discussion Mr. Craven called Mr. Hoover to confirm his relationship with Mr. Qualls and discussed some concerns.
On 1 June 2007, Mr. Hoover called again and informed me that Mr. Qualls had passed him a bad check, Exhibit “F”, and asked what he should do since he was in West Virginia and Mr. Qualls lived in Gallipolis, OH. He was concerned because the check was written from an account in Springfield, OH. I informed him to confront Mr. Qualls for his money. He stated the Mr. Qualls gave him a story that the check was dated wrong and to run it again. Mr. Hoover told Mr. Qualls that his bank told him the check would not run again due to insufficient funds.
On 3 June 2007, I met with Lisa Davern and Paul Hoover at 18240 Ohio River Rd, West Columbia, WV 25287, which is Ms. Daverns residence, and again Mr. Terry Qualls address on his OH/WV Bonding License. Ms. Davern has been a resident (renter) at this address since February 2007 this year.
Ms. Davern and Mr. Hoover informed me that sometime about 3 – 4 months ago, Mr. Terry Qualls asked Lisa Davern if he could use her address in West Virginia for mailing purposes. Lisa and Paul stated they were not aware that Terry Qualls was using their address to obtain a West Virginia Bonding License or for any other purposes other than to receive mail he wanted to keep from his girl friend (Jamie Toles). Mr. Qualls led Mr. Hoover and Ms. Davern to believe he sometimes got mail and surprise gifts for Jamie Toles and he wanted to use that address to keep the gifts for her a secret and a surprise.
I asked Lisa and Paul how they got connected with Terry Qualls and Lisa tells me that Terry came to her to inform her that he was hired by her husband to investigate her. Lisa is currently going thru a divorce. Paul has been with Terry for sometime, they even worked together in a Halfway House “Next Step” in Gallia County OH. The Halfway house went under due to financial problems. BTW, Mr. Qualls was not hired by Lisa’s husband but used that as a tool to get Lisa Davern’s trust. If he was hired by someone to investigate someone else then he would need a Private Investigators License (PI) or some authority to investigate. He does not have a PI license or any noted documentation/authority to run investigations.
On 4 June 2007, Mr. Hoover contacted the Municipal Court of Gallia County for information on how to process the bad check.
On 3 June 2007, approximately 1500 hrs (3:00 PM), I contacted the Gallia County Sheriffs Office and spoke to Officer Northup at the County Jail. She tells me that she remembered when Mr. Qualls came in with his cards and that one of the deputies on duty commented that Mr. Qualls had a warrant a few months back. He commented maybe the bonding company was pretty hard up for agents or didn’t know the individual.
On 4 June 2007 at approximately 0930 hrs (9:30 AM), I met with Ms. Noreen M. Saunders, Gallia County Clerk of Courts to discuss Mr. Terry Qualls. She was surprised that Mr. Qualls had obtained an Ohio Bail License and was hired to post bonds. For further information on Mr. Qualls you can contact Noreen M. Saunders Tel: 740-446-4612 Ext 224.
On 4 June 2007 at approximately 1015 hrs (10:15 AM), I met with Lou Ellen Werry - Municipal Court Clerk, Gallia County, to discuss Mr. Terry Qualls. She too was very surprised that Mr. Qualls had obtained an Ohio Bail License and was hired to post bonds. She stated she wasn’t so concerned about the court but more for the clients of Mr. Qualls. During my visit with Lou Ellen Werry - Municipal Court Clerk, Gallia County, other clerks at the municipal count began to comment on their knowledge of Mr. Qualls. Seems most all the City Court Officials know Mr. Qualls.

Lou Ellen Werry, Municipal Court Clerk, Gallia County ran a printout of Mr. Qualls history with her court, Exhibit “H”. Currently he owes $1842.55 in back fines and court cost, Exhibit “I”. Based on Court Records in Gallia County it appears that writing bad checks has been prevalent with Mr. Qualls in the past.
